Hospice
is a model of care for individuals who have a life-limiting illness. The focus is on palliative care, emotional closure and
personal peace for both patient and family. Hospice is not merely a service, but rather a mechanism that facilitates the patient
in his/her ability to process and accept the natural progression of life.
WHO QUALIFIES FOR OUR SERVICES?
Individuals become eligible
for hospice services by virtue of a physician certifying that they have a life-limiting illness. This eligibilty is not a
function of the patient's diagnosis, but a result of his/her prognosis.
WHERE CAN AGAPE HOSPICE SERVICES
BE PROVIDED?
Individuals who need and want hospice services may receive them at his/her
home, in a long term care facility or other appropiate settings of their choice.
DOES HOSPICE PROVIDE FOR ALL THE NEEDS OF THE PATIENT?
Hospice is responsible
for meeting all the needs of the patient that relate to the patient's medical situation. Services not related to the life-limiting
illness are the responsibility of other payor sources.
HOW
OFTEN WILL AGAPE'S STAFF VISIT THE PATIENT?
Agape's
staff will visit the patient and his/her family as often as necessary. The frequency and focus of the visits is contingent
on the patient's and family's needs. This schedule is outlined in the Hospice Plan of Care and is updated as needed on a basis
of not less than once a month. These updates will be used to monitor and reflect upon changes in the patient's condition.
